ROLE SHEET The purpose of the role sheet is to monitor daily attendance of clients. The role sheet is an important tool for analyzing absence trends, rewarding attendance, and confirming information if/when a refund is requested.
SIGN-IN SHEET The Sign-In Sheet serves several purposes. It creates a sense of client accountability, acts as a liability waiver, is used to collect client contact data, and ensures that all participants safely complete each class. Additionally, the sign-in sheet can be used by trainers to complete the daily role sheet.
The Liability and Consent Waiver ensures that clients take personal, legal responsibility for their participation in Fitness Bootcamp.
The waiver requires clients to confirm that they are healthy and physically capable of exercise. It also reminds them to seek medical advice if they are unsure and medical clearance if they have a pre-existing condition. This is an important tool in establishing that clients are personally responsible for their own participation and helps protect LDC and trainers from legal action in the event a client is injured or becomes ill during a Fitness Bootcamp meeting.
Purpose: Individual Assessment Forms are offered to clients to keep track of their progress during a session of Fitness Bootcamp.
Importance: Assessment is an important tool in any fitness program as adjustments must be made if/when a program is found to be ineffective. Without an assessment tool, there is no way to gauge where adjustments are needed.
Purpose: Group Assessment Sheets help trainers keep track of client progress for the purpose of recognition, pairing of participants, and development of teams.
Importance: This form is optional and offers trainers an ‘At a Glance’ view of the entire group
The purpose of the Sub-Contract Instructor Agreement is to provide an understanding and agreement between the company (LDC Training) and independent contract instructors.
The Sub-Contract Instructor Agreement is an important tool of reference for both the company and team members. The agreement is a vital tool used to outline instructor duties, pay rate, hours, and benefits. This tool helps avoid misunderstandings and conflict which might arise otherwise, and helps preserve an amicable relationship between the company and team members.
The purpose of the Time Sheet is to provide an actual accounting of hours worked and a record of monthly payment.
Although the Trainer’s Contract outlines the hours and payment agreed to, the Time Sheet provides for the time actually worked. Ideally, both forms will reconcile, but occasionally actual time worked will deviate from the contracted time. Examples include: trainer illness/absence; holidays; vacation; expos; covering other trainers; etc..
